Principles of eyelid reconstruction:
Full thickness lower eyelid defect
Final Note:
While it is tempting to reconstruct any lower lid defects. Sometimes spontaneous granulation (lassiez-faire) may produce a better result than any reconstruction can achieve. This is especially true if the lower lid defect is shallow. The following pictures illustrate such cases.
Lower lid defect following Moh's procedure
for diffused basal cell carcinoma. The defect
involves half of the height of the tarsal plate
The appearance of the lower lid three weeks later. Despite being left to granulate spontaneously, the contour of the lid is good. The lack of eyelashes in the lower lid is of no great cosmetic consequence compared with their losses in the upper lid.
The eye closure is satisfactory without ocular exposure.
