Answer b Internal carotid artery aneurysm within the cavernous sinus. Answer b Possible ocular signs relate to compressive lesion: a. sixth nerve palsy (this is affected earlier than other nerves supplying the extraocular muscles as it traverses the cavernous sinus). b. Horner's syndrome (the sympathetic plexus travels along the wall of the internal carotid artery which can be affected by the aneurysm).
