a You are asked to perform ocular motility on this patient. What is the most likely diagnosis? Answer The patient has a frosted lens over the right eye  suggesting that she is suffering from diplopia.  The eye behind the lens is ptotic and down and  out making third nerve palsy the most likely  diagnosis. See picture below for more information. g   a The right eye is down and out with ptosis and a dilated pupil. The diagnosis is a right surgical third nerve palsy.  This patient has an inoperable cerebral aneurysm. Return to observation II
